The Chicago Dental Society Foundation Clinic provides dental care to low-income residents of DuPage, Cook and Lake counties. The Chicago Dental Society Foundation is the only dental foundation that owns and operates a clinic in Illinois run by all volunteer dentists.

Mediation & Peer Review Service
Chicago Dental Society members always strive to provide you with the best possible oral health care services. Sometimes, however, services don’t go as expected. In those instances, CDS offers a free, and strictly confidential mediation service to help solve those issues, without the expense of a court case.
- The first step is for a patient to download and complete the Request for Review of Dental Services Form.
- Working with Illinois State Dental Society (ISDS), CDS then assigns the request to members of the CDS Mediation and Peer Review Committee for their review.
- Committee members are dentists volunteering their mediation and peer review services. They may ask to interview or examine the patient and talk with the dentist who provided treatment.
- The entire process is free and strictly confidential. Dentists choose whether or not to participate.
- In some cases, CDS is able to resolve complaints to both parties’ satisfaction.
- This service is available to member dentists of the Illinois State Dental Society. Find out if the dentist is a member.
As the ISDS oversees the program, patients must first complete the Request for Review of Dental Services Form and mail it to:
